Senior Case Management
Family Service Agency’s Senior Case Management Program helps older adults connect with the resources they need to live safely, independently, and comfortably in their own home or community.
FSA can assist with accessing your basic needs essential for daily living such as
- adequate nutrition
- health care
- health insurance
- in-home support services
- mental health counseling
- abuse prevention
- transportation
Thanks to funding from the Modernization of the Older Californians Act and the Area Agency on Aging, the service is free to adults 60 and older. This service is offered in English and Spanish.
